ReFocus (“we,” “our,” or “us”), a HIPAA covered entity, respects your privacy and is committed to protecting the personal information you share with us in compliance with the HIPAA Privacy and Security Rules, as well as applicable state laws in New York, New Jersey, Connecticut, Massachusetts, Pennsylvania, Rhode Island, Virginia, Delaware, and Maryland. This Privacy Policy explains how we collect, use, disclose, and protect your information when you visit our websites or interact with us online, including anonymized data collection practices.
Information We Collect: We may collect personal information (name, email, phone), non-personal data (browser type, IP address, analytics), and limited health-related details if you submit them through our forms. Any Protected Health Information (PHI) submitted is used solely for responding to your inquiries, anonymized where possible, and handled in a manner consistent with HIPAA regulations, including encryption in transit and at rest via SSL/TLS. No marketing trackers are used on the site.
How We Use Information: To respond to requests, schedule appointments, improve services and site performance, communicate updates (with consent where required), and comply with legal obligations such as HIPAA risk assessments and state privacy laws. We retain personal information for as long as necessary for these purposes, generally 6 years for PHI under HIPAA standards.
Sharing: We do not sell or rent personal information. We may share information with service providers who support our operations (e.g., hosting, analytics, communications) under confidentiality obligations and executed Business Associate Agreements (BAAs) as required by HIPAA. Sharing occurs only with authorities when required by law, and all data is anonymized to the extent feasible.
Security: As a covered entity, we use reasonable administrative, technical, and physical safeguards including SSL/TLS encryption for PHI, multi-factor authentication where applicable, and annual risk analyses to protect information per the HIPAA Security Rule updates effective 2025. In the event of a breach, we will notify affected individuals within 60 days (or 30 days per updated rules if applicable) as required by HITECH. No method of transmission or storage is 100% secure; we cannot guarantee absolute security.

Third-Party Services: Our sites may use third-party tools (e.g., maps, embedded videos) under BAAs where PHI is involved; their collection and use of data are governed by their own policies, but we ensure HIPAA alignment. No children's data under 13 is knowingly collected (COPPA compliance).
Changes: We may update this Privacy Policy periodically to reflect 2025 HIPAA and state law evolutions; the “Effective Date” reflects the latest version. Last updated: October 28, 2025.

Any disputes shall be resolved through binding arbitration in Delaware, waiving class actions to the extent permitted.
